A Lasting Power of Attorney appoints someone to make decisions
on behalf of a person who is unable to do so for themselves. When
completing a Lasting Power of Attorney form, make sure you
understand the form and fill it out correctly.
There are four parts to a Lasting Power of Attorney form:
- the information sheet
- part A – to be completed by you
- part B – to be completed by your ‘certificate providers’
- part C – to be completed by anyone you have named as an
attorney
The forms can be filled in without getting professional legal
advice. However, if there are complicated details (such as
finances) or specific instructions for the attorneys, you may want
to get legal advice.
